feat: Update bump-version.sh to handle CHANGELOG.md

- Automatically moves [Unreleased] section to [version] - date
- Creates new empty [Unreleased] section at top
- Includes CHANGELOG.md in version bump commit
- Follows Keep a Changelog format properly
