Add .gitignore to .beads directory during init (#64)

- Create .gitignore file in .beads/ when running bd init
    - Ignores *.db and *.db-* patterns to prevent database commits
    - Add test coverage to verify .gitignore creation
    - Add .claude/settings.local.json to project .gitignore
