Remove ASCII tree characters and use cleaner indentation with

- handleClose now emits MutationStatus with old/new status
- handleUpdate detects status changes and emits MutationStatus
- Add comprehensive tests for rich mutation events

- Fix silent error handling in --follow mode (bd-csnr):
