chore: clean up root directory cruft
- Delete duplicate install.sh (scripts/install.sh is canonical)
- Delete BD-3S8-CHANGES.md (implementation now in git history)
- Delete .saved-stashes/ (3 obsolete patch files)
- Move internal dev docs to docs/dev-notes/:
- ERROR_HANDLING_AUDIT.md
- MAIN_TEST_CLEANUP_PLAN.md
- MAIN_TEST_REFACTOR_NOTES.md
- TEST_SUITE_AUDIT.md
