Add gt convoy add command for adding issues to existing convoys (gt-nq3pr)

Implements the 'add' subcommand to convoy that:
- Validates the convoy exists
- Automatically reopens closed convoys
- Adds 'tracks' dependency for each issue
- Warns but doesn't fail on issues that can't be added
