docs(identity): fix stale polecat identity description
Update identity.md to reflect the implemented polecat identity model. The previous text incorrectly stated "Polecats are ephemeral... no persistent polecat CV" which contradicted the polecat-lifecycle.md docs and the gt polecat identity implementation.
